How do blind or visually impaired internet users handle passwords?

Thinking about accessibility on the web, this article explores some of the problems (and potential solutions) for handling passwords safely via screen-readers for blind or visually impaired people:

The author, James Edwards, struggles to find a good solution for handling passwords with screen-readers that is genuinely usable concluding: “Accessibility is not an exercise in meeting abstract checks, it’s about improving usability for people with specific needs . So a solution that’s technically accessible but is not usable for these groups of users, can’t really be considered accessible at all.”

It’s an interesting read and I wonder if others know of other ways passwords are handled by websites in ways that are accessible for screen-readers and the visually impaired?
